Join our System!
We are excited for you to join our system. This process is for new EMTs or Paramedics who will work under our system's guidance and will allow new providers to obtain privileges to perform care.
- Download and complete the Personal Information Form
- Ask your EMS Coordinator to schedule your system entry date with Justin Williams contacting him at justin.williams@nm.org or 630-933-6910
- Ensure your "Letter of Good Standing" and all CE has been sent to the EMS office at Central DuPage Hospital (CDH EMS, 25 N Winfield Rd., Winfield, IL 60190)
- If you arrive after the scheduled start time, you will not be allowed to test.
SYSTEM ENTRY DATES
System Entry will be held twice a month at CDH. Please review the information in the calendar to confirm start time, dates, locations. If you have any questions about dates, locations, etc., please reach out to Justin Williams.
Letter of Good STanding
If you are an EMS provider in our system and need a letter of good standing you must request it in writing to the EMS office utilizing the Letter of Good Standing request form. Please review the policy for the L-3 Letter of Good Standing Request.
The request should be sent to cdhems@nm.org.
Note: The letter will not be provided to you directly and only sent to person / organization that you specified in your request. Please allow up to 10 business days for your letter to arrive.

Updating personal Information
You must submit all of your agency personnel information changes to the EMS office at Central DuPage Hospital. Your EMS coordinator should send it to Justin Williams..
